Can a woman leave her house without being accompanied by a guardian from her family?

Fatwa Question or Essay Title: 
Can a woman leave her house without being accompanied by a guardian from her family?
She can go to nearby places without a mahram (guardian, see glossary) while observing hijab and provided there is no fear of fitnah (evil/mischief). But for a journey, she should be accompanied by mahram.
